Assisted Funeral Service Application Form

Before proceeding to the Assisted Funeral Services Application confirm the following have been contacted if it applies to the deceased:

  • Financial Services Commission of Ontario if the deceased was involved in a motor vehicle accident and not insured at time of death, or 
  • Automobile Insurance company if the deceased person was insured and involved in a motor vehicle accident at time of death
  • Victim Service of Brant if the deceased was a victim of death due to a crime of violence
  • Veteran Affairs Canada if the deceased was a veteran
  • Workplace Safety and Insurance Board if the death was a result of a work-related injury
  • Department of National Defense if the deceased was a member
  • Indigenous Services Canada if the deceased was a member of a First Nations Band living on the Six Nations of the Grand River reserve
  • A fraternal organization, club or lodge if the deceased was a member
  • Sponsor of the deceased person who had status of a Sponsored Immigrant at time of death
  • Funeral Provider if deceased was single at the time of their death and owned property
  • Home for Special Care if the deceased was a resident in a Home for Special Care at the time of death
